Unsubscribed from Waiting List look-up values

After updating to version 60, service providers may have randomly become unsubscribed from some look-up values. This is particularly present in Waiting Lists, although it's unclear if the issue is present elsewhere.
For example, at Service Provider #1, when I look at Waiting Request Reason, I see that everybody is now unsubscribed from all values.
The issue is also present for Waiting List Status.
If you want to add a client to a waiting list, there's no options in the drop-downs:
And for an existing client, it would remember their previous value but you can't select a different one.

I think both of these lookup tables were new in 59, previous to that version they could not be customized. I saw this in our v60 test environment as well where nothing was available for Status and Reason in the waiting list module after upgrading from 59 to 60.
However, when I checked my 59 production, nothing was actually subscribed even though all the values from both Status and Reason appeared in the Waiting List module. So I think what happened here is that they added the lookup tables in 59, but missed something with creating subscriptions and reading them back for display, which was fixed in 60.
